The album (vinyl double LP set) was released in the UK where it was called 'Made In Japan'. - It was released in a gold laminated gatefold sleeve on the 'Purple Records' label. It did not contain picture / lyric inner sleeves or any inserts and certainly no 'negative'.
The same album was also released in Japan where it was re-titled 'Live In Japan'. This double album had the same track listing and was exactly the same recording as Made In Japan but had a completely different gatefold sleeve and initial pressings included the film negative insert (together with the usual 'obi' and Japanese lyric insert). Copies of this Japanese version of the album were available in the UK at the time as a very expensive import. Popular belief for a while was that it was a different live album. Those who shelled out for it soon discovered that it was, in fact, the same as Made In Japan. (Actually, it might even have been imported before the UK version was released.)
When the album was made available in the US and in the rest of the world, all territories (as far as I know - probably the far east used their own sleeve design..?) used the UK 'Made In Japan' gold sleeve design.
I bought the last issue of Deep Purple minis (not the current one but the one before that - 2006?) [Ed: this release]. At that time this album was released with two different sleeves included in the one package - both the Japanese one with the negative insert plus a spare (empty) sleeve of the original UK design. (Actually the US sleeve as all of these were based on the US Warner Bros. Albums, not the UK Harvest / Purple Records albums - but the gold gatefold sleeve is basically the same).
So... the negative actually belongs with the Japanese version of this album which is called 'Live In Japan' and has a unique sleeve design. As far as I know the negative insert was included with all copies of this mini and all copies should have come with two different sleeves.
